#31: Due to severe apical inflammation, bone grafting only was planned to preserve the alveolar ridge, postponing implant placement.
#32: Immediate placement of a Neobiotech S-Narrow Ø3.2×11mm implant after extraction, combined with socket grafting to enhance bone stability.
Final Prosthesis: A pontic restoration is planned for #31 to maintain esthetics and function.

Case Presentation
1️⃣ Pre-Op
A fistula was observed in the apical area of #31, indicating chronic infection.
Periapical radiography showed an extensive periapical lesion at #31, making immediate implant placement infeasible due to the severity of the infection.
The surgical site was prepared for extraction with an assessment of the surrounding soft tissue and alveolar bone condition. Extraction was performed with minimal surgical trauma, followed by thorough debridement of all remaining inflammatory tissue to create an optimal bone environment for implant placement.
2️⃣ Surgery - S-Narrow Implant
The implant was placed using the S-Narrow Kit, with direct application of the Ø2.7 drill for initial drilling.

Before implant placement, D011 bone quality (hard bone) was observed, and pre-tapping was performed to facilitate implant insertion.
Threading of the alveolar bone was achieved through pre-tapping, followed by implant placement while tactile resistance of the threads was assessed.
An S-Narrow Ø3.2×11mm implant was immediately placed at #32, with insertion torque adjusted from an initial 60 Ncm to a final 30 Ncm.
An S-Narrow implant was placed subgingivally after extraction, with socket grafting planned to fill the remaining gap around the extraction socket and implant to enhance bone stability.
Alveolar bone grafting was performed using Neo Bone (allograft) and DM Bone (alloplast).
After bone grafting, a Ø3.0 Healing Abutment with 3mm Cuff Height was placed to complete the procedure, and a flipper was provided as a temporary prosthesis for functional and esthetic purposes.
3️⃣ Post-Op
The 1-month follow-up intraoral photography shows stable healing of the soft tissue around the healing abutment.
4️⃣Final Restoration
At the 4-month follow-up, the final restoration was completed, and intraoral photography shows the prosthesis in place, allowing for the evaluation of both functional and esthetic outcomes.

5️⃣ Follow-Up
The 6-month follow-up shows stable esthetics and function, with no signs of inflammation or bone resorption.
The 3-year follow-up image shows no signs of inflammation or alveolar bone resorption, with stable esthetic and functional maintenance.
